1st printing: July 2002, 5½ x 7", beige cover, (printing indicated in the indicia), "maybe 30-40 copies" mostly sent to reviewers**
2nd printing: April 2003, 7 x 8½", yellow cover (printing indicated in the indicia), "maybe 100 copies" **

** According to publisher Russ Maheras, who also says: "There are also about 6-7 ashcan versions with white cover stock, side-stapled, and 8.5 x 11 in size, and they have 'first printing' on the indicia."

No date of publication (circa 1970), and no indication of who published it.  It appears to be a bootleg, but it’s also been said that it was published by Russ Cochran.
